Anyone can ask MHLW to switch a prescription ingredient to OTC. Since August 2016 there have been 83 requests: 20 later switched, 11 rejected (the council voted go/no-go until 2020), 46 still open. Since March 2021 the evaluation council no longer votes; it publishes issues and countermeasures, and a company then files an OTC application that goes to the PAFSC OTC subcommittee. Latest council meeting: #36 on 2026-05-22. Next OTC subcommittee (closed session): 2026-10-02 — approval of OTC product Riup Regene X5 (minoxidil) (agenda, PDF).
Why it matters here: once an OTC version launches, the prescription version can become “OTC-similar” and fall under the 1/4 charge — subcommittee members have raised this concern. 6 of the 83 requests are for ingredients already on the list.
